FM2028 Surrender RCG competency: declaration by NSW competency card holder

If you wish to undertake Licensee training and hold a current RCG competency endorsement that you wish to surrender, you should complete this form. This surrender will apply even if you recently gained or refreshed your RCG endorsement.
