  2. Confirmation medical report/s -  Get ready reports to confirm your diagnosis and need for lifelong attendant care support when accessing the community. You can use free online tools to reduce your report file sizes. Scan and save in PDF format ready to upload.
  3. Coloured passport-style photo - Get ready a passport style photo. You can use your phone and free online tools to resize and reduce file sizes. Attach as a JPG file.
