Priority vaccination registration – Privacy collection statement
This Privacy Collection Statement is to advise you of the personal information the Department of Customer Service (the “Department”, “we” or “our”) will collect from you as a result of your registration for a priority COVID-19 vaccination appointment via www.nsw.gov.au.
Any personal information the Department collects from you in your use of the other features of nsw.gov.au will be handled by the Department in accordance with the Privacy Statement for the NSW Government website.
In order to register for a priority COVID-19 vaccination appointment, you will be required need to complete the registration form available via https://www.nsw.gov.au/covid-19/health-and-wellbeing/covid-19-vaccination-nsw/childcare-and-disability-workers.
The registration form requires you to provide certain personal information.
The personal information you are required to provide when you register for a priority vaccination appointment includes:
- your name
- your phone number
- your email address.
You will also be asked to confirm that you are a childcare worker or disability support worker and that you live in an area of concern.
The Department is collecting your personal information for the purposes of working together with NSW Health to register you for a priority COVID-19 vaccination appointment.
You are not required by law to provide the requested personal information to the Department. However, if you do not provide your personal information to the Department you will not be able to register for a priority vaccination appointment.
The Department may use and disclose your personal information to NSW Health in relation to your registration for a priority COVID-19 vaccination appointment. NSW Health may contact you, including via text message or email, to provide you with a unique booking code to book an appointment.
The Department will not provide your personal information to third parties for any purposes not already stated in this privacy collection notice, or to which you have not otherwise consented to, unless the Department is required, or authorised, by law to do so.
The Department will hold your personal information for 30 days, after that time your information will be securely deleted.
