Complete the fee waiver application form

In some circumstances, fees for some services may be waived under the Registry’s Fee waiver policy or where otherwise approved by the Registry.

A fee waiver may be requested for services including, but not limited to:

• birth certificates (including the integrated birth certificate for adopted people)
• death certificates
• marriage certificates
• change of name registration and certificates
• change of sex registration and certificates
• recognised details certificates.

Before you download

Before you complete the fee waiver application, check that the service you need can be considered for a fee waiver.

You should also prepare  supporting evidence for the fee waiver circumstances that apply to you. The fee waiver application outlines what supporting evidence you need to supply.

After you fill out the form

When you have completed this application and prepared the required documents, add them to your Registry application.

Submit all documents according to the instructions on the application form.
