Registry records are confidential. We can only provide information and/or certificates to people who are permitted under the Registry’s Access policy.
If you are entitled to apply for information and/or a certificate but you can’t submit an application yourself, you can choose someone else to do it for you. This person is called an authorised agent.
An authorised agent could be a family member, friend, carer, casework practitioner, solicitor or anyone acting in a professional capacity.
Note: Legal guardians and people with power of attorney do not need a letter of authority. They should provide a copy of their power of attorney or guardianship order instead.
To choose your authorised agent, you need to complete the letter of authority below.
Before you complete the letter of authority, make sure you are eligible to apply for the information and/or certificate you want first.
Once you have confirmed your eligibility, you need to gather proof of identity documents for both you and your authorised agent.

When you have filled out the form, please add it to your Registry application, along with:
