Complete the original registration documentation form

Registry records are confidential. We can only provide original registration documents to people who are entitled to receive information under the Registry’s Access policy.

While the Registry keeps records of life events registered in NSW, the original registration or application documents may not always be available.

If you are not entitled to access the information, do not have a sufficient reason for your request, or if the requested document is no longer held by the Registry, your application will be declined and the fee refunded.

Before you download

Once you have confirmed your eligibility, you need to prepare your proof of identity documents and supporting evidence.
