Thriving with new and innovative models of care

It's been almost a year since the Community Management Centre (CMC) moved into the new Prince of Wales Hospital (PoWH) Acute Services Building (ASB). The team is making a huge difference, since emerging during the challenging time of the COVID-19 pandemic.

The South Eastern Sydney Local Health District service, hosted by PoWH, began as a way to care for COVID-19 patients in the community. 

Managing patients since 2021, the CMC team have become experts in the hybrid nature of their work, expanding the service in March 2023 to include Heartlink, a heart failure patient service. 

Using a hybrid model of virtual monitoring and face-to-face care, the team consists of nurses, doctors and remote patient monitoring officers to build individual care plans for each patient. 

From the comfort of their home, the patient completes their heart reading and enters it into the online software, which they have been taught to use during their initial consult. 

CMC Acting Nurse Manager, Francis Nicolas said the transition into the ASB was a positive one. The move was entirely smooth sailing, with no interruption to patient care, and the new space continues to be beneficial every day.

The CMC service is delivered by a small yet dynamic team, offering a range of different skill sets and experience.

The Prince of Wales ASB is part of the Randwick Campus Redevelopment project.
