We work with children and their families in Hunter New England LHD and deliver services and support for their wellbeing in a connective and supportive way.
About our youth services
Some young people do not have the support they need to access proper health care. This can mean missing the care they need when experiencing difficulties or knowing what the right type of care is.
Our services focus on supporting young people by helping them to engage effectively with health and other services.
Youth Engagement Service
The Youth Engagement Service (YES) works with young people, aged 12 to 17 years, and their families or carers to connect them to the right support at the right time.
We work with young people, aged 12 to 17 years, after an emergency department presentation or a hospital admission.
Sometimes we also see young people who are at risk of developing or with worsening mental health concerns, if they do not have the right interventions.
Our purpose is to make sure existing services are not duplicated. To do this, we work together with other health services to support with engagement and provide linkage when there are some barriers.
Our services include:
- brief interventions for young people
- brief interventions for family and carers
- support to transition young people into the right services.

The YES team is made up of the following clinicians:
- clinical leader
- Clinical Nurse Consultant
- clinical Psychologist
- dietician.

Youth Health Team
The Youth Health Team is a free and confidential health service that works with young people aged 12 to 18 years.
Priority is given to young people who are homeless, at risk of homelessness, or have limited support from family or caregivers.
We support young people who frequent, live, work or study in the regions of Newcastle, Lake Macquarie, or Port Stephens.
Our team works to improve the health outcomes of young people with complex health and social needs.
We use a strengths-based, harm minimisation approach that respects and supports the young person to take responsibility for their health care.
We assist and encourage young people by providing:
- health awareness
- education and support around physical, sexual health and wellbeing
- guidance to independently manage their health and wellbeing
- access to appropriate healthcare and other services.
We offer young people a confidential health and wellbeing assessment, exploring key areas of their life to better understand their needs.
From there, we walk alongside the young person to set personalised goals, empowering them to take charge of their health in a way that works best for them.

If you or someone you know is in a crisis situation or needs urgent medical attention, call 000 (triple zero) or go to the nearest Emergency Department.

Youth Drug and Alcohol Clinical Service
The Youth Drug and Alcohol Clinical Service (YDACS) is a specialist service that works with young people, aged up to 18 years, experiencing moderate to severe drug or alcohol use.

The YDACS is flexible and can be designed to fit an individual young person's needs.
We understand that all young people come from diverse backgrounds, genders, sexuality, cultures and have their own unique experience and stories.
Some people start using alcohol or drugs to feel good or manage difficult emotions, which may have impacts on their mental health, like:
- heightened anxiety
- depression
- psychosis
- finances (in debt)
- attendance at school or work
- relationships with family and friends
- overall health (weight loss, sexual problems).
Using alcohol or drugs can lead to some unwanted and very harmful effects. Often people decide to reach out for help when they or someone close to them can see their use is causing harm.

Your healthcare rights and privacy information
Everyone seeking or receiving healthcare in NSW has certain rights. Learn more about your healthcare rights and patient privacy.
- First appointment - We'll talk to you about all aspects of your life. As part of getting to know you, our team:
- will ask questions about your physical and psychosocial health including your substance use and about your family
- may conduct a physical examination, blood work and catch-up on your vaccinations on-site, only if needed.
- Second appointment and clinical review - We complete your assessment and goal planning. The YDACS team will conduct a clinical review, discuss and plan how we can best support you around your:
- substance use
- life and health
- emotional regulation techniques.
- Meet with other clinicians - You'll meet with other members of our clinical team as needed and review any additional concerns.
- Work on your goals - You'll keep seeing your lead clinician and continue to work on your life goals.
- Review progress - We'll schedule a mid-way appointment with you to review your progress and goals. Our team will conduct a clinical review and make any adjustments needed to best support you.
- Discharge appointment - Our team will meet to review your plan and prepare for your discharge appointment. We'll meet with you to go over your journey, plan any onward referrals and your discharge plans.
We encourage you to ask us any questions or discuss anything you would like to address while you're with us.

Families and carers can use the same number and email for information and support.
We understand that families and carers of young people are their greatest strength and best support. Whenever possible, we make sure families are included in the assessment and treatment.

Any service provider can make a referral to our service, including:
- police
- schools
- GPs (General Practitioners)
- community health services
- inpatient hospital units
- Department of Communities and Justice.

Transition Care Service
Transition is when you move from paediatric (child & young people’s health services ) to adult health care.
It is the process of getting ready and transferring to the adult health care system which typically happens when you leave school or around 18 years.
Within the Hunter New England Local Health District, a team is available to assist young people and their families with the transition process. For young people who may never be fully independent, tailored support is available to ensure families and caregivers receive the guidance and resources they need.
The Transition Care Service helps young people (14 to 25 years) with chronic health conditions to prepare to manage their health care more independently, including:
- Increasing self-management skills, adjust to new services, attend healthcare appointments, and take on more responsibility for their own care when appropriate.
The service also supports young people & carers by:
- Assisting with transition planning from children to adult health services and liaising with services
- Finding adult healthcare services to suit young people’s needs.
- Supporting families & carers helping their young person transition.
- Attending first adult health care appointments and transition clinics if required.
A young person with chronic health conditions (14-25 years) who:
- lives in NSW Central Coast, Hunter, New England, mid north or northern NSW
- has a chronic health condition or disability (excluding a primary diagnosis of mental health conditions) that needs complex case coordination to transition to adult specialist health services.
The young person’s family or carer.
Healthcare professionals, schoolteachers, case workers and others who support the young person with the young person’s permission.
