Patients experiencing or at high risk of foot ulcers, infections or certain other conditions of the foot can access specialist appointments, tests and procedures without being admitted to hospital.
About us
The High Risk Foot Service is a multidisciplinary clinic which includes podiatrists, pedorthist, diabetes nurse educator and endocrinologist, with access to vascular and orthopaedic surgeons, and infectious diseases specialists.
We provide access to specialist multidisciplinary care in an outpatient setting for people with diabetes related foot ulcers and infections.
Referral information
You will need a referral from a GP, specialist, podiatrist or registered nurse.
The completed referral form can be faxed to us, addressed to Dr Zheng (Endocrinologist) or to the High-Risk Foot Service. GPs and referring clinicians can access the form in the Nepean Blue Mountains LHD outpatient referral catalogue.

Referral criteria
The patient must reside in the Nepean Blue Mountains Local Health District and have one or more of the following:
- active or suspected Charcot arthropathy
- suspected osteomyelitis with or without a foot ulcer
- foot ulcer with or without infection
- critical limb ischemia
- dry gangrene
Urgent referrals
The following clinical conditions are considered urgent:
- active or suspected Charcot neuroarthropathy
- suspected osteomyelitis with or without a foot ulcer
- critical limb ischaemia.
Call us immediately if you need urgent review, or visit the Emergency Department.
Clinic services, specialists and hours
Once we receive a referral, we will contact you to arrange an appointment at our clinic, which runs Monday to Wednesday 8:30am to 4pm.
