Patients with memory or cognitive disorders can access specialist appointments, tests and procedures without being admitted to hospital.
About us
This clinic sees patients with memory or cognitive concerns who are over 70 years of age (55 years for Aboriginal or Torres Strait Islander people) or over 65 years if they have multiple chronic and complex issues.
Referral information
The patient will need a detailed referral letter from their GP to be faxed to the clinic, including family or carer contact details, and we will contact them to arrange an appointment.
The patient must have the following pathology tests done before their appointment:
- vitamin B12
- folate
- thyroid function (TFT)
- full blood count (FBC)
- kidney function (EUC)
- liver function (LFT)
The Concord informant questionnaire for diagnosis of dementia needs to be completed by either the patient or a carer before an appointment with a memory clinic.
