About the Government Information Access application form

Complete this form to apply for formal access under the Government Information (Public Access) Act 2009 (the GIPA Act) for Government information held by NSW Treasury. If you need help to complete this form, contact the Information Access and Governance Unit at gipa@treasury.nsw.gov.au.

Questions that are marked with an asterisk (*) are required.

There is an application fee of $30. If you are given access to the information you are looking for, you may be asked to pay a processing charge of $30 an hour beyond the first hour. Some applicants may be entitled to a 50% reduction in their processing charges, details of the discount are available in the form.

We are collecting your information to process your request to access information under the Government Information (Public Access) Act 2009 (GIPA). To process your request, we require certain information from you, as outlined in Part 4 of the GIPA Act, for example: your name and your postal/email address.

We may share your information with another agency which holds or is likely to hold the information you have requested. We will not share it with anyone else unless you give your consent, or the law requires or allows us to do so.  

Are you required by law to give us your information?

You are not required by law to give us your information, but if you choose not to, we may not be able to process your request to access information under the Government Information (Public Access) Act 2009 (GIPA). To process your request, we require certain information from you, as outlined in Part 4 of the GIPA Act, for example: your name and your postal/email address.
