You can refer yourself to many community health services.
Some services need more information about your health before your appointment. This may include a referral from your GP or another health professional.

If your situation is urgent, tell us when you call or include this in your referral.
Tips for referrers
- Provide the patient’s full name, date of birth and medical record number (MRN). Also include their current address.
- Check and include the best phone number for the patient or their nominated carer or support person.
- Central Intake may need to contact you to clarify the referral. Include your name, role and contact number.
- Clearly state the service requested, for example physiotherapy, occupational therapy or women’s health. Provide information using the ISBAR format where possible.
- Do not include multiple patients in the same email. Referrals with more than one patient will be returned and will need to be resubmitted separately.
- Include keywords in the subject line that reflect the referral type and priority. If the referral needs action within 24 hours, mark it as “Priority Processing”.
- Save your referral form as a PDF and email it to SNSWLHD-CommunityIntake@health.nsw.gov.au. You will receive a confirmation email.

Services not referred through Central Intake
The 1800 011 511 Mental Health Line is NSW Health's 24/7 statewide phone service which links people with NSW Health mental health services. It is a free service.
My Aged Care can help you find and access government-subsidised Australian aged care services. You can call My Aged Care on 1800 200 422.
For enquiries or to make a dental appointment you can phone 1800 679 336, Monday to Friday, 8am to 4:00pm (excluding public holidays) or Live Chat with our team.
Southern's Hospital in the Home (HITH) service provides acute clinical care in the home, in clinic settings, and through virtual appointments.
To arrange a transfer or direct admission to one of our hospitals or our Hospital in the Home service, call our District Patient Flow Unit on 1800 999 880.

Southern's District Patient Flow Unit coordinates patient movements between hospitals in Southern NSW Local Health District, and to or from hospitals outside our district.
The unit manages transfers, coordinates conference calls between referring and accepting clinicians, confirms bed availability, and books patient transport so care is timely and safe.
