About our heart and lung care services
We provide diagnostic and ongoing care for patients who are at risk of, or who have been diagnosed with respiratory and cardiac diseases.
We aim to support people to maintain and improve their quality of life by building long-term self-management skills. This includes techniques to help clear sputum and reduce feelings of breathlessness over time.
The conditions we treat include:
- bronchiectasis
- Chronic Obstructive Pulmonary Disease (COPD)
- asthma
- pneumonia
- post-sternotomy
- rib fractures.
This service is based at Blacktown Hospital.

About the service
Cardiology in Community (CIC) is a partnership program between Western Sydney Primary Health Network (WSPHN), Western Sydney LHD and general practices in Western Sydney.
The program aims to support and increase detection and management of heart conditions to prevent serious illness and admission to hospital.
How it works
CIC helps identify and manage heart conditions earlier by linking people with coordinated care from their GP, specialist teams and community services.
If your GP invites you to join, it means they believe the program could support your heart health. You may have screening tests, wear a heart monitor or attend regular check-ups. Based on your results, you’ll receive a personalised care plan that may include specialist or pharmacist support.
The program is free, with GP visits bulk-billed during participation. You can stay in the program for as long as it benefits you, and you can leave at any time. CIC aims to give you faster access to care, better communication with your care team and more support to manage your condition. When the program ends, your GP continues your usual care.
