About our adolescent and youth health services

- disadvantaged or at risk
- homeless or at risk of homelessness
- marginalised (oppressed or underrepresented).
Our services focus on helping with:
- preventing harm and injury
- physical health and nutrition
- mental health
- sexual health
- alcohol and other drug use, including tobacco.
Patient privacy
All patients' personal information that we collect is held securely in accordance with privacy laws and policies that apply to NSW Health. Ask our staff about our duty of care and learn more about privacy of patient information.

Enquiries and referrals
Our youth health services are for young people who frequent, live, work or study in WSLHD (Blacktown, Cumberland, Mount Druitt, Parramatta, and The Hills).
You can send your referrals to High Street Youth Service or the Western Area Adolescent Team. We accept self-referrals and support-referrals, with the young person's consent, from:
- counsellors
- family members, friends and parents
- GPs (doctors)
- other service providers
- school teachers.
No Medicare card is required (except for oral health clinic and pathology).
In the interests of ongoing health care, we ask that referrals from NSW Health service providers include relevant supporting health information. Examples include:
- discharge summary
- current medication(s)
- assessments
- treatment and safety plans.

What we do
We support young people, no matter their age or background. Learn more about the different types of support pathways we offer.
Youth health works in all our hospitals' Emergency Departments (ED) and help disadvantaged young people reduce the number of times they present to the ED.
Care navigation focuses on young people without a mental health diagnosis or who do not meet mental health service criteria. We do this by:
- reducing their reliance on the ED
- improving their access to primary and community health care
- connecting them to the right healthcare at the right place and time.
The right healthcare options can include:
- acute specialist services
- community health services
- mental health
- NDIS supports
- primary health services.
The CAT team provides health care coordination, health education, and health promotion.
Case management
Each client receives an individualised client-led case plan that focuses on the social factors of their health including:
- accommodation
- advocacy (backing and support)
- education
- financial security
- food security
- other related health needs.
Early intervention
We provide different health education groups to help deliver a harm minimisation message. The groups focus on key health areas like:
- alcohol and other drugs
- mental health
- sexual health
- wellbeing groups.
The wellbeing groups are focused on young people who may not have access to general services and supports and miss out on important health messages and information.
No matter your age or background, we support young people with a range of mental health illnesses and disorders. We provide counselling to young people who are:
- at risk
- disadvantaged
- homeless or at risk of homelessness
- oppressed or underrepresented.
Young people who experience these setbacks often are isolated, have poor supports and histories of neglect and/or trauma. This can increase the risk of mental health issues and suicide.
Clients who come to counselling have a range of issues, including:
- behavioural problems
- inability to regulate emotions
- mood swings
- problems with family relationships
- self-harm
- substance use
- suicidal thoughts
- violent tendencies.
Our services include in-depth nursing and medical health assessments and preventative primary health care, including:
- contraception
- health and medical screening (as needed)
- immunisation
- limited primary medical care
- nurse-led health care support and navigation
- referrals
- Sexually Transmitted Infections (STI) testing.
In-reach nursing support
We also support the Wellbeing and Health In-reach Nursing (WHINS) program.
We provide speech pathology services to young people aged 12-24 years within WSLHD.
The Middle Years Project (project) recognises the 'middle years' (generally from 10 to 15 years) as a critical time for students who experience cognitive, emotional, physical and social challenges.
The service focuses on young people who:
- are at risk
- are oppressed or underrepresented
- are disadvantaged, homeless or at risk of homelessness
- are at risk of leaving or not engaging from the education system
- have difficulty engaging and socialising in their communities because of communication challenges.
Project partners
The project is a partnership between Western Sydney LHD and the following NSW government agencies:
- NSW Department of Education
- NSW Department of Communities and Justice
- NSW Police
- WentWest
For more information about this project email: SCHN-WS-SDR@health.nsw.gov.au

The WHINS program provides in-reach nursing in several primary and secondary schools in Western Sydney. Our nurses are co-located in the schools and work with the school's wellbeing, learning and support teams.
The program is based on the belief that:
- improved access to health care will improve student health and wellbeing
- improved student health and wellbeing support improved educational engagement and outcomes.
The WHINS program is delivered in partnership between NSW Health and NSW Department of Education.
We currently have 6 WHINS who partnered with the following primary and high schools.

The WoD service acts as a gateway to our adolescent and youth services. Any young person who walks through our doors can ask our WoD for support and information.
There is no wrong door or way, our WoD will work to support anyone who contacts our service within our health services or with external agencies or programs.
How to access the service
Young people can access the service through primary or secondary referrals via:
- e-referral
- telephone
- physically coming into our service location.
Note: Medicare cards are not required, all services are free.
What we do
We will:
- conduct an initial assessment
- use a triage (prioritising) system to connect you with the right service at the right time.
This service helps to plan, prepare and move (transition) young people with chronic health conditions or disabilities from child to adult healthcare services.
Our person-centred, co-ordinated approach prioritises each person's developmental and psychosocial needs. This improves health outcomes for young people and:
- prevents them from disconnecting from health services
- reduces avoidable hospital visits and admissions.
How to access this service
The Transition Care Network or Trapeze can provide transition coordination services for young people in Western Sydney.
If you are a health professional and need information about transition care for young person under your care, email WSLHD-HighStreet@health.nsw.gov.au

Your feedback matters
Your experience matters. If you have been a child or young person (patient), carer or family member at one of our youth health services, tell us what you think of our services. Share your comments and feedback with our patient and carer experience team by emailing WSLHD-CGMail@health.nsw.gov.au
My Experience Matters online survey
The patient experience rating is calculated from the responses our patients, carers and families provide via our My Experience Matters online survey. The questions cover topics that are important to you.
All responses are anonymous and only takes a few minutes to complete. If you have visited more than one youth health service, you can take the survey for each service you have visited.
