This form is for people who have requested to have their penalty decided in court.

To have the matter determined by a court, you will need to complete and submit the court election form. Please visit Revenue NSW for details.

If you are unable to pay the penalty amount by the due date, you can set up a payment plan to pay over a longer period by contacting Revenue NSW or submitting a payment plan application. This form can be downloaded from Revenue NSW. Alternatively, please call Revenue NSW on 1300 138 118.

If you want to dispute or seek leniency for a penalty, you can seek leniency based on certain circumstances or if you believe an error has been made. To request a review, please visit Revenue NSW.
