Program objective
- Reduce alcohol and other drug use related harm by delivering client-centred, high quality and safe diversionary activities, case management and care coordination.
- Improve the health, wellbeing and social outcomes of clients by delivering comprehensive alcohol and other drug support interventions that include the following clinical care standards:
- intake
- comprehensive assessment
- care planning and referral to clinical treatment services
- identification, responding to, and ongoing monitoring of risk
- monitoring treatment progress and outcomes
- transfer of care.
- Clients’ access to and engagement with alcohol and other drug, health and social services to meet their individual needs is improved.
This program is funded and administered by Nepean Blue Mountains Local Health District.
