Program objective
To provide outreach support to young people (aged 16-24) with a lived experience of moderate to severe mental illness, with the aim to:
- develop their strengths and skills to live independently and manage their own mental health
- support and encourage the recovery of young people participating in the program
- facilitate access of young people to specialist services to support individual recovery.
- commit to quality improvement and innovation in the provision of outreach support.
This program is funded and administered by Nepean Blue Mountains Local Health District.
