Get Healthy Service privacy statement

We collect and hold your information, including personal information, health information and general information to deliver services to you, and to ensure those services are evaluated for effectiveness.

We will only collect this information once you have provided consent to do so and have agreed to join the program.

How we collect your information

In most cases, we collect your information directly from you when you engage with us, from another health professional or clinician such as your GP, or from your authorised carer, all with your consent.

We won't use your information for any purpose without your consent to use it in that way except where we are permitted or required to do so at law.

We may collect, hold, use, and disclose your information for the following purposes, including: 

• to enable you to access and use the Get Healthy Service
• to send you service, support and administrative messages, reminders, technical notices, general updates, security alerts, and information requested by you
• to seek your feedback to help us make improvements to the Get Healthy Service, for research and evaluation, and to respond to feedback provided by you.
• for research and evaluation purposes. Without your consent no identified information will be shared.
• to comply with laws and assist government or law enforcement agencies where we are required and authorised to do so.

Even once you've provided your consent for a particular use or for future use, you can withdraw it at any time.

We may share your personal information with partners and for other reasons we tell you about in this policy. 

Please note that by participating in the Get Healthy Service you consent to the use and disclosure of your personal and health information in accordance with this Privacy Statement.

We may share your personal information with:

• NSW Health
• our employees
• third party suppliers (for example, research and evaluation institutes) and service providers. We require that all external parties sign a confidentiality agreement that requires them to comply with privacy laws and this Privacy Statement.
• specific third parties authorised by you to receive information held by us such as a family member or health professional involved in your care.
• other entities, including government agencies, regulatory bodies, and law enforcement agencies, if required, or permitted by law.
• another provider should there be a change in Get Healthy Service and a different provider is appointed.

We take a number of measures to keep your information safe and secure. This includes but is not limited to the following:

• We generally hold personal information in electronic databases but also store personal information in hard copy form and in various third-party databases and platforms.
• The Get Healthy Service website, portal and working environment are built with integrated physical, electronic, and managerial processes designed to safeguard your information and protect it from misuse, interference, loss and unauthorised access, modification, or disclosure.
• We train our staff about how to keep your information safe and secure.
• We have special processes, procedures and strong system controls in place designed to hold information securely.
• We have regular system testing to ensure your information remains safe at all times.
• We take reasonable steps to ensure that our partners to whom we disclose your information, protect it to the same standard we do using equivalent measures and safeguards.
• We only keep your information for as long as we need it or are lawfully required to keep it.

• You can request a copy of your information in writing at any time.
• You can ask to correct or update your information if you believe it is incorrect. In some cases, we may add further information to your account to note your views.
• If you wish to limit the information provided to the Get Healthy Service you may do so, however, that may limit the ability for the Get Healthy Service to fully support you.
• You can express your concerns or complaints to the Get Healthy Service about your privacy or the way we are handling your information. We take your concerns seriously and will seek to resolve any issue or concern as soon as possible. Please refer to concerns and queries section for the Get Healthy Service contact details.
• If you choose to withdraw before any coaching or advice has been provided the information may be deleted if requested by you in writing.
• Once records of you are held for the required period, they are then securely destroyed.

We will need to verify your identity when you request access to your personal information. We will aim to respond to your request within 10 business days.

If you're not happy with the way we handle your query or handle your information, you may choose to lodge a complaint with the Office of the Australian Information Commissioner (OAIC) by visiting the OAIC website.
