Expression of Interest for a Consumer Representative on Chronic Care Programs
The Far West LHD is calling for Expressions of Interests (EOIs) for a consumer representative to be part of its Integrated Care for People with Chronic Conditions (ICPCC) and Leading Better Value Care (LBVC) programs.
'It is vital to ensure that the committee has representation from a variety of stakeholders and therefore I wish to encourage people from the community to express interest in joining the Steering Committee for these two programs,' said Georgina Drewery, District Nurse Manager ICPCC and LBVC.
'The role is to represent the community with a focus on lived experience through the local hospital system with one or more chronic conditions such as heart or lung disease, diabetes, osteoarthritis, kidney disease, cancer care, falls and hip fracture,' she said.
The aim of the Steering Committee is that it will support current Far West LHD ICPCC and LBVC initiatives to be embedded as business as usual in all LHD sites.
The steering committee reports to the Far West Local Health District Board Quality Committee. Members will be required to attend a one hour meeting either in person or via video/ teleconference every 2 months.
'This is an exciting opportunity to be part of a team that will be working towards continuous quality improvement projects to improve patient and family experience and outcomes,' said Ms Drewery.
'The criteria is that you share the vision of the Local Health District and that you are committed to the involvement of patients and their families as an advisory consumer voice to engage in health care planning and delivery.'
