This step is completed by medical gas technicians

Your licence must refer to medical gas technician work.

What information you need to provide

Types of medical gas tested

Select all of the medical gas types you have tested. If the gas type is not listed, select Other and provide details of the gas type.

Testing performed

Provide a brief description of the testing you have performed in the Comments box.

Commissioning start and end dates

Please note you cannot select a future date. The tests must have been completed when you submit this certificate.

Medical gas contractor details

If you are working for a medical gas contractor, enter their name and lience number.

Supporting documents

Click upload files and select the file(s) from your device to attach any supporting documents.

Once the file is uploaded, it'll be listed on the screen. For supporting documents, there are a maximum limit of 100MB per file and only the following formats are accepted:
