Entering facility details
What information you need to provide
Facility address
Add the address of facility where the work was performed. This can be done using the address search or by entering the address manually.

Facility or building name
Enter the name of the facility or building. This is the specific identifier within the site, for example:
- Building C
- A Block
- Main hospital
- Queen Elizabeth Building.
Facility type
Select the facility type from the drop-down list.
Customer contact details
Enter the customer's name and email address. This is the person who will receive the customer copy when the Certificate of Compliance is complete.
Include the builder's details here if the work is for a builder rather than a hospital or facility.
Entering facility address
Using the address search ensures your facility address is saved in the correct format. However, sometimes an address may not appear in the search so it will need to be entered manually.
- Start by typing the facility address in the address search box.
- You can use a facility name (e.g. Wollongong Hospital) or enter the street number and street name.
- Select the correct match from the drop-down list.
- Once all details are entered, select Save and continue to proceed to next step.

If no match appears in the address search drop-down list, select the Enter address manually option.
Enter the required fields, including lot, plan type, plan number, street name, suburb and postcode. All required fields must be completed.
The lot number and plan type (DP or SP) can be found on:
- the most recent rates notice from the property owner
- searching for the address in the NSW Planning Portal spatial viewer.
- Add extra details if needed (e.g. section, unit, apartment, street number or local government area.)
- Check the details are correct, then select Save and continue to confirm.
