This form can be used when you want to give consent for a social housing provider to exchange personal information with a nominated third party, and/or when you authorise a person or organisation to act on your behalf.
You can complete a consent form online without needing to attend an appointment. This online form is sent directly to your nominated social housing provider when you complete and submit the form.
Who this form is for
You can use this form if you:
- have already applied for social housing but have not heard back yet, or
- are already living in public housing (a tenant of Homes NSW).
Use this form to give permission for:
- someone to provide us information about you, or
- someone to act on your behalf.
What you’ll need to complete this form
To complete this form, you’ll need:
- your email address
- your mobile phone number
- either your Australian driver licence, Australian passport or Medicare card
If you’re including a person to act on your behalf
- The email and mobile number of the person/agency that will act on your behalf.
What you might also need
You might also need:
- your Homes NSW Client Reference Number, Centrelink Reference Number or Payment Reference Number.
