About the Independent Living Skills Assessment form
Use this form to update us if your ability to live independently has changed.
You can request completion of an Independent Living Skills Assessment Form electronically without needing to attend an appointment. This online form is sent directly to your nominated healthcare professional/support provider by email for their completion and then is returned to you for your review before submitting to Homes NSW.
Who this form is for
If you:
- have already applied for social housing but have not heard back yet, or
- are already living in public housing (a tenant of Homes NSW)
and you need to update us on your ability to live independently, you should use this form.
What you will need to complete the form
You will need:
- your email address
- your mobile phone number
- either your Australian driver licence, Australian passport or Medicare card
- your healthcare professional’s or support provider’s email and mobile phone number.
What you might need to complete the form
You might need:
- your Homes NSW Client Reference Number
- your Centrelink Reference Number, or
- your Payment Reference Number.
What you might need if you included an assessment request for someone else
If you have included an assessment request for an additional adult and are not their legal guardian, you may need:
- their email address
- their mobile phone number.
