About the medical assessment form
If a medical condition or disability may affect your housing needs, use this form to let us know.
You can request to complete a Medical Assessment Form online without needing to attend an appointment. This online form is sent directly to your nominated healthcare professional/support provider by email for their completion and then is returned to you for your review before submitting to Homes NSW.
Who this form is for
You can use this form if you:
- have already applied for social housing but have not heard back yet, or
- are already living in public housing (a tenant of Homes NSW).
Use this form to tell us about any medical conditions or disabilities that:
- you have
- someone in your household has that may affect your housing needs.
What you’ll need to complete this form
To complete this form, you’ll need:
- your email address
- your mobile phone number
- either your Australian driver licence, Australian passport or Medicare card
- Your healthcare professional’s or support provider’s email and mobile phone number.
What you might also need
You might also need:
- your Homes NSW Client Reference Number, Centrelink Reference Number or Payment Reference Number.
If you’ve included an assessment request for an additional adult
If you've included an assessment request for an additional adult and you are not their legal guardian, you might also need:
- their email
- their mobile phone number.
