What this form is for
You can use this form to:
- report a change in Income (including Centrelink, wages or child support)
- report a change in cash savings, shares, or other assets worth more than $5,000
- add or remove people from your household
- give us consent to pay your tenancy accounts using Centrelink payments
- give us consent to look at the Centrelink account details for you or your household
- report a change to whether you or someone else in your household owns:
- land
- a dwelling
- other property
- provide us with details of a medical condition through a Medical Assessment
- provide us with an Independent Living Skills Assessment
- notify us that you are leaving your tenancy.
