This form is for tenants of:

• The Aboriginal Housing Office

who want to tell us about changes to their tenancy.

It is also for tenants we have contacted about a rent subsidy review.

What this form is for

You can use this form to:

• report a change in Income (including Centrelink, wages or child support)
• report a change in cash savings, shares, or other assets worth more than $5,000
• add or remove people from your household
• give us consent to pay your tenancy accounts using Centrelink payments
• give us consent to look at the Centrelink account details for you or your household
• report a change to whether you or someone else in your household owns:
  • land
  • a dwelling
  • other property
• provide us with details of a medical condition through a Medical Assessment
• provide us with an Independent Living Skills Assessment
• notify us that you are leaving your tenancy.
