Incident Reporting Procedure

The purpose of this procedure is to provide steps for employees to complete factual, detailed incident reports in line with the requirements of the Incident Management Policy.

The steps to be taken by staff depend on the role of the staff member. These roles include: Unit Manager, Assistant Unit Manager, Shift Supervisor, Assistant Manager, Duty Manager, admissions staff, and all staff.

The steps to be taken include:

  • completing a staff report
  • completing an incident advise
  • SAP reporting
  • governance and approval of reports.
