Case Management Policy

The Youth Justice NSW Case Management Policy provides a framework for delivering consistent and effective case management to young people involved with Youth Justice NSW. It emphasises a collaborative, strengths-based, and culturally responsive approach that empowers young people to actively participate in decisions about their goals and support needs. The policy outlines key stages of case management, including assessment, case planning, implementation, monitoring, review, and exit planning, with a focus on reducing reoffending and supporting positive life outcomes. Case plans must be developed in partnership with young people, reflect their strengths, needs, culture, and aspirations, and be regularly reviewed to ensure they remain relevant and effective.

The policy also highlights the importance of ongoing assessment, family and community involvement, referrals to external services, and coordinated case conferences to support goal achievement. It requires regular case plan reviews, strong record keeping, and management oversight to ensure quality and accountability. Transition and exit planning are key components, helping young people prepare for changes such as release from custody or the end of supervision by linking them with ongoing supports and building their skills, confidence, and independence.
