A standard relationship certificate can be used to help prove your identity and to apply for certain services. 

A commemorative certificate is a decorative certificate only and is not a legal document.

You can also use this form to apply for a revoked relationship certificate.

Before you complete the relationship certificate application, make sure you are eligible to apply. 

Once you have confirmed your eligibility, you need to prepare your proof of identity documents and supporting evidence.
