Relationship registration application form

If you are not married, you can use this form to apply to legally register your relationship as ‘de facto’. 

If you order a certificate in your application, you may be able to use it to access some entitlements and services. For more information, visit the relationship register webpage.

Note: couples in registered relationships are subject to certain obligations and/or restrictions under NSW law. 
 

Before you complete the relationship registration application form, make sure you are eligible to apply and are in the presence of an authorised witness for the declaration. 

Once you have confirmed your eligibility, you need to prepare your proof of identity documents and supporting evidence. 
 

When you have completed the application and prepared the required documents, submit according to the instructions on the application form.
