Access Application Form for Premier's Department

Complete this form to apply for access to government information held by the Premier's Department under the Government Information (Public Access) Act 2009.

The Department has a Christmas closedown period pursuant to Premier’s Memorandum M2023-02 Christmas Closedown for the years 2023-2026 from Thursday, 25 December 2025 – Friday, 9 January 2026 inclusive. Please note this web page will not be monitored during this time.

The Premier's Department now accepts payment of the $30 GIPA application fee by credit card. Please contact us on (02) 9228 5871 if you are unable to pay by credit card. 

Before you start your application, please remember

  • You may not need to request a formal access application. The information may already be available or we may be able to give it to you through an informal request. Please contact us or check our website to find out if the information you want is already available.
  • After you submit the application, you will be emailed a copy of the form.

  • Identification documents, such as driver’s licence, Medicare card or passport (if you are seeking personal information).
  • Contact address and phone number.
