Program objective
The program, Discharge Planning and Transfer of Care for Consumers of NSW Health Mental Health Services, ensures:
- minimum standards to support effective and safe discharge planning and transfer of care for consumers of NSW Health mental health services
- consistent, coordinated approach to ensure continuity of care and support for the consumer and for their family/carers at the point of transfer of their care.
- Clarity of role and responsibility of mental health services in discharge planning and transfer of care including their linkages with other health care providers and support services, to meet the needs of mental health consumers and their family/carers.
Southern NSW Local Health District is responsible for the conduct of public hospitals and health institutions and the provision of health services for residents of the areas in respect of which it is constituted.
The Home Based Outreach Service is a program designed to deliver community based psychosocial support for people with a mental illness in the Queanbeyan/Cooma, Goulburn/Yass Valley and Bega Valley Shire Local Government Areas (LGA).
The program is designed to enhance the existing State Wide Housing & Accommodation Support Initiative (HASI).
This program is funded and administered by Southern NSW Local Health District.
Eligibility
Who can apply
Applicants must
- be a registered non-government organisation (NGO) and approved Mental Health provider with a legal entity registered in Australia
- and have appropriate financial capability.
The LHD will not enter into an Agreement with a company that does not have an Australian Business Number and is not registered for GST.
This preference supports the required internal NSW Health system set up that will enable access to and the raising of purchase orders for Contract items within the NSW Health procurement system.

Evaluation criteria
Applicant must provide details of relevant experience and past performance in the provision of recovery based psychosocial services in the community.
Responses should clearly demonstrate
- the number of years they have been in business under their present constituted form
- summary of current locations and range of services delivered
- experience working with the target client population
- capacity to identify and meet the changing support needs of clients with Mental illness
- strategies and experience in provision of culturally safe and appropriate services for Aboriginal people, including an Aboriginal Impact statement
- strategies and experience in engaging and providing services to other vulnerable populations (including people from multicultural communities, people experiencing or at risk of homelessness, LGBTIQ+, older people)
- effective pathways and linkage for clients to other programs and services they may need including coordinated referrals to employment, education and training, NDIS, aged care
- an understanding of local community needs and capacity to be flexible, agile, and innovative in the development of services to meet individual client needs
- outcomes of service evaluations
- applicant must provide an outline of the proposed location(s) and staffing model relevant to each region applied for in the application.
Governance, Leadership, and ability to implement
Applicant must provide an outline of
- corporate management structures, including decision making authority matrix covering personnel and site management, and where this program fits within this structure
- staff recruitment and selection processes, clinical supervision, credentialing, retention and appointment systems
- systems for continuing education, learning and professional development and training programs, and the range of programs provided
- incident and risk management systems
- robust service evaluation and monitoring processes
- quality assurance, accreditation, internal controls and evidence of alignment with (or working towards) the National Standards for Mental Health Services and/or the NSW Disability Services Standards
- a system for reporting complaints, compliments and identifying opportunities for improvement in service delivery and practices
- risk management policy and procedures including pathways for escalation of care, when required
- experience in the establishment, implementation and/or transition of services or clients to your organisation
- a proposed Transition In and Out Plan including actions and estimated timeframes.
Partnerships, Risk Management
Applicant must describe how they will
- ensure collaboration
- establish and maintain effective partnerships
- share governance arrangements with Southern NSW Local Health District.
Include examples of where this has been successful previously in NSW Districts or equivalent in other jurisdictions.
Applicant must describe a proposed structure and process for shared decision-making on
- consumer needs
- care and treatment relating to high quality individualised care planning and review
- quality of care provided
- safety (including the keeping of relevant documentation and record sharing between partners and sub-contractors where relevant).
Applicant must describe how they will manage the risks outlined in ‘Attachment A - Risk Management’ and demonstrate relevant skills and experience to do so.

- The qualifications and experience of key personnel that would be involved in the operation of the Proposed Deed.
- An outline of proposed staffing to client ratios and staff mix to support this program.
- Details of any specific, existing organisational resources available to support people in this program ie value added expertise and/or roles within the organisation.
- Current strategies, policies and frameworks for consumer and / or carer participation, including current role diversity and positions that exist for peer support workers.

- The LHD will evaluate the applications in accordance with the evaluation criteria to determine the overall value for money outcome for the LHD.

- Applicants are advised to respond clearly to all the evaluation criteria listed.
The LHD may exercise discretion in relation to the evaluation of the application, including when evaluating any subjective evaluation criteria.
