Youth on Track Discretionary Referral Form for police, Youth Justice and solicitors

This form is used by police, Youth Justice and solicitors to make a discretionary referral of a young person to the  Youth on Track program. The young person will be screened by an officer from Youth Justice to assess their eligibility for the scheme.
