About the Companion Card
The NSW Companion Card program is for people with significant and permanent disability who have a lifelong need for attendant care support when accessing the community. The Companion Card allows a cardholder’s support person free entry into participating venues and events.
How to apply
Check your eligibility
To be eligible for a NSW Companion Card, you must:
- Be a person with significant and permanent disability; and
- Be unable to participate at most community activities without attendant care support due to the impact of your disability.
- This level of attendant care will be life-long; and
- You are an Australian citizen or permanent resident and live in NSW.

PRINT - PART B: Health Professional Declaration:
Take this form to your treating medical practitioner. Scan and save in PDF format ready to upload.
REPORTS - Get ready copies of recent reports.
These reports should confirm your diagnosis and need for lifelong attendant care support when accessing the community.

PHOTO - Get ready a passport style photo.
You can use your phone or free online tools to turn your photographs into passport size. Save in a JPG format ready to upload
Make sure your photo is:
- high quality colour
- full front view of your head and shoulders only
- taken against neutral background
Incomplete applications will not be assessed.
