Complete the correct an entry application form

Registry records are historic documents. We can only amend records and certificates to reflect information that was correct at the time of the recorded life event under the Registry’s Corrections policy. A correction cannot be made to reflect more recent circumstances, such as a change of name.

Note: marriage celebrants and funeral directors should not use this form. Please use the specific business partner forms to submit a correction request.
