Program objective
- Reduce alcohol and other drug use related harm by delivering client-centred, high quality and safe case management and care coordination.
- Improve the health, wellbeing and social outcomes of clients by delivering comprehensive case management and care coordination that include:
- intake
- care planning
- identification, responding to, and ongoing monitoring of risk
- transfer of care.
- Support improved client access to, and engagement with, alcohol and other drug, health and social services to meet their individual needs.
- Reduce the need for acute inpatient hospital admissions by managing people's wellness and retention in outpatient treatment services as well as engagement in community based support services with regard to adequate housing, income, food and clothing.
This program is funded and administered by Nepean Blue Mountains Local Health District.
