SHARE SMR provides physical activity group sessions to people over 50 years of age and residing within the South Eastern Sydney Local Health District (SESLHD) geographic region. These ad hoc programs are locally funded by South Eastern Sydney Local Health District (SESLHD).
SESLHD monitors each program’s performance throughout the 12 month term of the grant agreement.
Quarterly or biannual progress reports are required with detailed information on agreed key performance indicators and targets, including financial performance, as set out in the NSW Health Grant Agreement.
Data provided by the NGO is closely monitored and used to evaluate the activity.
Regular meetings and visits to the service are scheduled to provide both parties an opportunity to discuss any performance related issues or concerns.
SESLHD acts on findings of their evaluation by providing feedback or working with the NGO to improve outcomes.
A review and evaluation is completed at the end of the grant period to assess whether the program achieved expected outcomes and whether there is demonstrated ongoing community need for the programs.
