Program objective
These ad hoc grants are locally funded through South Eastern Sydney Local Health District (SESLHD), which is committed to partnering with the non-government sector to deliver important community-based services that support the health and wellbeing of the public, especially vulnerable or hard to reach populations. These grants are also administered through South Eastern Sydney Local Health District.

Program evaluation
These ad hoc programs are locally funded by South Eastern Sydney Local Health District (SESLHD).
SESLHD monitors each program’s performance throughout the 12 month term of the grant agreement.
Quarterly or biannual progress reports are required with detailed information on agreed key performance indicators and targets, including financial performance, as set out in the NSW Health Grant Agreement.
Data provided by the NGO is closely monitored and used to evaluate the activity.
Regular meetings and visits to the service are scheduled to provide both parties an opportunity to discuss any performance related issues or concerns.
SESLHD acts on findings of their evaluation by providing feedback or working with the NGO to improve outcomes.
A review and evaluation is completed at the end of the grant period to assess whether the program achieved expected outcomes and whether there is demonstrated ongoing community need for the programs.
