Discretionary referral form – Youth on Track 2022

Referrals to Youth on Track can be made by organisations working with young people who have had a formal contact with the police, along with displaying a range of risk factors. Use this form to make a discretionary referral for a young person.

The Youth on Track referral form is used to make discretionary referrals to the program.

These referrals are made by organisations that work with young people and have identified someone that they think is a high risk.  

People who can make discretionary referrals include: 

  • NSW Police 

  • Justice Health and Forensic Mental Health Network 

  • out-of-home care providers 

  • the Family Referral Service 

  • mental health services and organisations 

  • solicitors.
