Declaration by competent person (for a tenant’s dependent child)

This form is for having a 'competent person' declare that the tenant's child is a victim of domestic violence perpetrated by the 'relevant domestic violence offender'. Use this form in conjunction with a Domestic Violence Termination Notice to end a tenancy early due to domestic violence.
