The Care Leavers' Charter of Rights is written by young people who have previously left out-of-home care.
The Charter describes what caseworkers, members of the Department of Communities and Justice and other non-government agencies involved in a young person's care can do to help them transition smoothly into adulthood.
It also outlines a young person's rights in out-of-home care and how to communicate their needs clearly.
