About the program
Home and Healthy supported homeless people and those at risk of homelessness leaving health facilities, including hospitals by offering holistic support. The aim of the program was to support participants during their transition from a health facility then ongoing for up to two years with an allocated case manager and housing strategies.
The program was delivered as a trial by Mission Australia in the Sydney and South Eastern Sydney Local Health Districts (LHDs).
Where we are now
The Home and Healthy was not extended after its pilot period. Performance reviews found that the number of Home and Healthy service users achieving sustained housing, and employment and training outcomes was lower than expected, and the characteristics of the Home and Healthy service users were different from the cohort for whom Mission Australia designed the program.
After collaboratively exploring possible refinements to the target cohort and other program changes, all parties agreed to not extend the program beyond its pilot period.
