Work experience application form
We encourage local high school students to get a taste for a health care career through the Health Inspirations and Koori Kids Futures work experience programs.
Before you apply
Please be aware of the following prior to applying:
- Only students attending school and/or residing in the Nepean Blue Mountains Local Health District are eligible to participate. This includes the Penrith, Hawkesbury, Blue Mountains or Lithgow local government areas.
- Applications received from outside of the Local Health District will not be considered and will not receive a response from the Committee.
- Completion of the application form does not guarantee a place in the program.
- This application will be reviewed by the Committee.
- All eligible applicants will be notified in writing about the result of their application.
- All correspondence will be conducted through School Careers Advisors, Aboriginal Education Officer or School Principals.
- Successful applicants will be required to:
- obtain consent from their parent or guardian
- complete the Health Inspirations attendance form, including media and image consent, medical management plan (if applicable) as well as a permission for long-term follow-up.
- sign the Student Agreement form
- complete the Student Placement Record, as required by the school.
- Places are strictly limited. Students who are not successful will need to re-apply for subsequent programs.
Only students meeting the following criteria can apply for our 5-day Health Inspirations work experience program:
- high school students who either live or attend school within the Nepean Blue Mountains Local Health District (Penrith, Hawkesbury, Blue Mountains or Lithgow local government areas)
- currently in year 10 or 11
- interested in one or more of the following:
- medicine
- nursing or midwifery
- psychology
- mental health
- radiography
- occupational therapy
- speech pathology
- dietetics
- physiotherapy
- finance, human resources, communications, information and technology.
Only students meeting the following criteria can apply for our 5-day Koori Kids Futures work experience program:
- Aboriginal or Torres Strait Islander high school students who either live or attend school within the Nepean Blue Mountains Local Health District (Penrith, Hawkesbury, Blue Mountains or Lithgow local government areas)
- currently in years 10 to 12.
