Work experience application form

Please be aware of the following prior to applying:

• Only students attending school and/or residing in the Nepean Blue Mountains Local Health District are eligible to participate. This includes the Penrith, Hawkesbury, Blue Mountains or Lithgow local government areas.
• Applications received from outside of the Local Health District will not be considered and will not receive a response from the Committee.
• Completion of the application form does not guarantee a place in the program.
• This application will be reviewed by the Committee.
• All eligible applicants will be notified in writing about the result of their application.
• All correspondence will be conducted through School Careers Advisors, Aboriginal Education Officer or School Principals.
• Successful applicants will be required to:
  • obtain consent from their parent or guardian
  • complete the Health Inspirations attendance form, including media and image consent, medical management plan (if applicable) as well as a permission for long-term follow-up.
  • sign the Student Agreement form
  • complete the Student Placement Record, as required by the school.
• Places are strictly limited. Students who are not successful will need to re-apply for subsequent programs.
