TOP 5
Sharing your knowledge about your loved one’s behaviour can help our staff better understand the needs of the person they’re caring for during their hospital stay or in their home. We use information called TOP5 to help personalise care for patients, particularly those with memory, thinking and communication problems.

Carer Gateway
The Carer Gateway provides online and phone information and support for carers across Australia.

Resources & Support for Carers
National Disability Insurance Scheme (NDIS)
The National Disability Insurance Scheme (NDIS) is Australia’s first national scheme for people with disability. It provides individualised support for people with permanent and significant disability, their families and carers.

My Aged Care
My Aged Care is the Australian Government’s contact centre for older people, as well as their families and carers, to access aged care services.

Young Carers NSW
Young Carers are children or young people, 25 years or under, who help or take on the caring role in their family. This program supports and connects young carers providing information, referrals, counselling and workshops.

Reportable Conduct Scheme
The NSW Reportable Conduct Scheme monitors how organisations investigate and report on allegations of certain conduct towards children by an employee.
